CareMed is seeking a Nurse Practitioner to deliver primary and preventive care to residents in long-term care facilities. You will manage acute changes, participate in advanced care planning and chronic care management, and coordinate with a multidisciplinary team.

Prescribe medications per Alabama law, order tests, and collaborate with primary care physicians to optimize resident health outcomes within an interdisciplinary team.

Formación

• Current, unrestricted NP license in the state of Alabama.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
• Psych experience a plus.

Responsabilidades
