Job summary

Creative Solutions In HealthCare in Texas is hiring a Full-Time Physical Therapy Assistant (PTA) to join our in-house rehab team. You will deliver person-centered care to help residents improve mobility, strength, and quality of life under the supervision of a Physical Therapist.

The ideal candidate holds an active Texas PTA license, has facility-based experience, and can communicate well with patients, families, and the interdisciplinary team.

Qualifications

• Active Texas PTA license required.
• Experience in SNF or long-term care preferred.
• Knowledge of physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Administer prescribed physical therapy treatments to relieve pain and improve function.
• Provide skilled PT services under supervision of a Physical Therapist.
• Relay information and referrals to the Physical Therapist.
• Implement and modify care plans under PT supervision.
• Measure and record motor function, strength, and muscle performance.
• Monitor patient response and adjust approaches with the interdisciplinary team.
• Provide education to patients, family members, and caregivers.
• Ensure documentation complies with policies and regulations.
• Attend meetings as designated by the Director of Rehab.
