Job summary

Creative Solutions In HealthCare is seeking a compassionate Full-Time Physical Therapy Assistant (PTA) to join our in-house rehab team. You will provide person-centered care to help residents improve mobility, strength, and quality of life under the supervision of a Physical Therapist.

The ideal candidate holds a Texas PTA license, is in good standing with regulatory bodies, and has SNF or long-term care experience.

Qualifications

• Active and valid Texas PTA license.
• SNF or long-term care experience preferred.
• Knowledge of physical therapy treatment practices.

Responsibilities

• Administer prescribed PTA treatments to relieve pain and improve function.
• Provide skilled PT services under supervision of a Physical Therapist.
• Respond to service requests and coordinate with the PT.
• Implement and modify plans of care under PT supervision.
• Measure and record motor function, strength, and performance.
• Monitor patient responses and adjust approaches with the team.
• Support patients, families, and caregivers with education.
• Ensure thorough, timely documentation per policies and regulations.
• Attend meetings as designated by the Director of Rehab.
