Job summary

Lock Haven Rehabilitation and Senior Living is seeking a compassionate Full-Time Physical Therapist (PT) to join our rehabilitation team. The PT will evaluate residents, develop individualized plans, and work with nurses, physicians, and families to promote mobility and independence.

Responsibilities include documenting progress, adjusting treatments, and educating residents and caregivers. Prior experience in skilled nursing or post-acute settings is preferred; PA licensure is required.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Active Pennsylvania Physical Therapist license in good standing.
• CPR certification preferred.
• Experience in skilled nursing or post-acute settings preferred.
• Experience with Net Health and PCC preferred.
• Strong clinical and interpersonal skills.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide PT interventions to improve mobility, strength, balance and independence.
• Monitor progress and adjust plans as needed.
• Document evaluations, treatments and progress notes timely.
• Collaborate with interdisciplinary team for care goals and discharge planning.
• Educate residents, families and caregivers on plans and safety techniques.
• Maintain compliance with policies, regulations and guidelines.
• Participate in care conferences and team meetings.
• Ensure a safe and therapeutic environment for residents.
