Job summary

Memorial Health System in Taylorville, IL is seeking a Staff Physical Therapist to provide and coordinate comprehensive care for patients of all ages. You will evaluate, plan, and implement treatment plans in line with established standards and Memorial Health System policies.

Qualified candidates are licensed or eligible for licensure in Illinois and CPR certified; a valid Illinois driver’s license is required.

Qualifications

• Graduate of an approved school of Physical Therapy.
• Illinois eligible or licensed Physical Therapist.
• CPR certification is required.
• Must possess valid Illinois driver’s license and must be deemed an acceptable driver in accordance with the MHS Fleet Safety Policy; five year MVR will be required and proof of automobile insurance.

Responsibilities

• Provide and coordinate comprehensive physical therapy care for patients of all ages.
• Evaluate, plan and implement individualized treatment plans.
• Adhere to established standards, policies, procedures, protocols and guidelines.
