BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a full-time or part-time opportunity performing home health visits for our senior living office, located in Moorestown, NJ. This office services adult and geriatric clients on a per-visit basis in territories throughout Burlington and Camden County, NJ.

One year of prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing, enabling clients to receive necessary medical care at home.

Responsibilities include:

• Make home visits to clients in designated geographic areas.
• Perform diagnostic tests and measurements, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
• Implement wound prevention and repair interventions using evidence-based dressings and topical agents.
• Develop and implement individualized care plans, including manual exercises, gait training, and balance interventions.
• Assess and revise care plans regularly, collaborating with nursing in multidisciplinary team meetings.
• Educate clients and their families on rehabilitative activities to promote health, safety, and independence.
• Document observations, interventions, and evaluations accurately using a touch pad tablet.

Qualifications:

• Current NJ Physical Therapist license.
• Graduation from an accredited program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with electronic medical records (EMR) preferred.
