Qualifications

Responsibilities

• Establishes and administers a treatment plan with therapy goals per the physician’s direction.
• Assesses patient needs and adjusts care to meet goals of therapy.
• Collaborates with all disciplines to plan and evaluate team goals.
• Sets realistic and achievable goals for patients.
• Documents patient condition and progress per regulations.
• Educates patients and families in therapy methods.
• Optimizes functionality by screening residents as directed.
• Communicates status and needs to patient, family and professionals.
• Ensures punctuality and regular attendance for assigned shifts.

Bachelor’s/Master’s/Doctorate in Physical Therapy
