Job summary

• Active PT license required.
• Strong communication and clinical decision-making skills.
• Skilled nursing experience preferred but not required.

Responsibilities

• Complete physical therapy evaluations and establish individualized treatment plans.
• Provide therapy focused on strength, balance, gait training, mobility, endurance, and pain management.
• Collaborate with PTA staff, nursing, physicians, and interdisciplinary team members to support resident care goals.
• Monitor resident progress and adjust treatment approaches as needed.
• Maintain accurate and compliant documentation.
