The role emphasizes establishing functional goals, outcomes, and discharge plans tailored to each resident’s needs. You will collaborate with an interdisciplinary team, support discharge planning for both long-term and short-term residents, and educate residents, families, and caregivers on care plans to ensure continuity

Qualifications

• Current state licensure in Physical Therapy, in good standing.
• Bachelor’s degree in Physical Therapy.

Responsibilities

• Develops and revises plan of care based on residents’ needs and sets functional goals, outcomes, and discharge plans.
• Plans for residents’ discharge and anticipates long-term and short-term needs.
• Educates health care team members, residents, family, and caregivers on the care plan.
