Responsibilities

• Develops and revises plan of care based on residents’ needs and establishes functional goals, outcomes, and discharge plans.
• Plans for discharge and anticipates needs for long term and/or short term residents.
• Educates other health care team members, residents, family, and caregivers on information regarding the care plan.
