Woburn Rehabilitation and Nursing Center in Woburn, MA is seeking a dedicated Physical Therapist (PT) for a full-time role. The position offers a $42–$58 per hour pay rate with a $10,000 sign-on bonus, joining an in-house rehabilitation team focused on improving patient mobility and independence.

The ideal candidate is licensed in Massachusetts, has strong evaluation and documentation skills, and thrives in interdisciplinary collaboration.

Qualifications

• Current Massachusetts Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program.
• Strong clinical assessment, evaluation, treatment planning, and documentation skills.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively as part of an interdisciplinary rehabilitation team.
• Commitment to providing compassionate, patient-centered care.
• Previous experience in skilled nursing or post-acute rehabilitation is preferred but not required.

Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans based on their clinical needs, goals, and functional abilities.
• Provide skilled physical therapy services to improve mobility, strength, balance, coordination, and independence.
• Implement treatment plans and modify interventions based on patient progress and response to treatment.
• Establish measurable short- and long-term rehabilitation goals.
• Collaborate with nursing, therapy, medical, and other interdisciplinary team members to support coordinated patient care.
• Communicate effectively with patients, families, caregivers, and facility staff regarding treatment plans and progress.
• Educate patients, families, and caregivers on therapeutic exercises, mobility, safety, and strategies to support continued progress.
• Complete accurate and timely documentation, including evaluations, treatment notes, progress reports, and discharge documentation.
• Participate in care planning and family conferences as appropriate.
• Maintain compliance with Massachusetts licensure requirements, facility policies, Medicare, and other applicable payer guidelines.
• Promote patient safety, dignity, independence, and quality of life throughout the rehabilitation process.
