Responsibilities

  • Evaluate patients and develop individualized physical therapy treatment plans based on clinical needs, goals, and functional abilities.
  • Provide skilled physical therapy services to improve mobility, strength, balance, coordination, and independence.
  • Implement treatment plans and modify interventions based on patient progress and response to treatment.
  • Establish measurable short- and long-term rehabilitation goals.
  • Collaborate with nursing, therapy, medical, and other interdisciplinary team members to support coordinated patient care.
  • Communicate effectively with patients, families, caregivers, and facility staff regarding treatment plans and progress.
  • Educate patients, families, and caregivers on therapeutic exercises, mobility, safety, and strategies to support continued progress.
  • Complete accurate and timely documentation, including evaluations, treatment notes, progress reports, and discharge documentation.
  • Participate in care planning and family conferences as appropriate.
  • Maintain compliance with Massachusetts licensure requirements, facility policies, Medicare, and other applicable payer guidelines.
  • Promote patient safety, dignity, independence, and quality of life throughout the rehabilitation process.

Woburn Rehabilitation and Nursing Center is seeking a dedicated and compassionate Physical Therapist (PT) to join our in-house rehabilitation team in Woburn, MA. This is a Full-Time opportunity for a licensed Physical Therapist who is passionate about helping patients improve mobility, regain independence, and achieve meaningful rehabilitation outcomes.

What You'll Do
  • Evaluate patients and develop individualized physical therapy treatment plans based on their clinical needs, goals, and functional abilities.
