Qualifications:

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License

Job Responsibilities include, but are not limited to:

• Evaluate, review, and screen patients'/residents' medical charts, evaluations under physician order, and develop a treatment plan according to patient needs.
• Implement treatment plans through direct treatment and educate team members about the plan of care, including supervising PTAs as necessary.
• Attend/Lead interdisciplinary meetings to relay clinical findings, make recommendations, and conduct staff/family training and education when appropriate.
