Job summary

Qualifications

• Licensed Physical Therapist (LPT) required in state of employment; BLS required.
• Bachelor's degree in a related field is required.
• Doctorate in Physical Therapy preferred.

Responsibilities

• Gathers data from chart and other care givers; discusses with physician as necessary.
• Independently assesses all patient types as assigned.
• Recognizes additional needs during evaluation and uses good judgment for testing and scheduling.
• Interprets results and formulates measurable, realistic goals.
• Reassesses patient progress regularly per policy.
• Identifies equipment needs and discusses with patient and family.
