Job summary

Pecan Tree Rehab and Health is actively seeking a full-time Physical Therapist (PT) to join our in-house team. You will evaluate patients, create individualized treatment plans, and deliver compassionate care aimed at improving mobility and quality of life.

The role emphasizes evidence-based practice, collaboration with an interdisciplinary team, and thorough documentation in line with state and federal regulations. Texas licensure is required, with SNF experience preferred.

Qualifications

• Active and valid Texas Physical Therapist license.
• SNF or long-term care experience preferred.
• Knowledge of current physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Consult with patients to assess their physical condition and functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans using a variety of therapy techniques.
• Create personalized rehabilitation programs tailored to each patient’s goals.
• Administer prescribed physical therapy treatments to relieve pain and restore function.
• Ensure treatments align with the established plan of care.
• Monitor patient response and adjust approaches with the interdisciplinary team.
• Provide education to patients, families, and caregivers.
• Collaborate with team to ensure high-quality care.
• Complete documentation in compliance with policies and regulations.
