Job summary

White Blossom Care Center in San Jose, CA is seeking a motivated Physical Therapist (PT) to join our rehabilitation team in a skilled nursing setting. We offer a comprehensive benefits package and a collaborative atmosphere dedicated to resident recovery and independence.

This full-time position provides a benefitted rate of $60–$65 per hour, with opportunities to treat residents, lead clinical training, and develop individualized rehabilitation plans.

Qualifications

• Education: Graduate of an accredited Physical Therapy program.
• Licensure: Current and active California Physical Therapist license (or eligibility for new grads).
• Skills: Ability to relate positively and effectively with residents, families, and professional colleagues.
• Integrity: Strong commitment to confidentiality and resident rights.

Responsibilities

• Evaluate residents to determine physical therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders.
• Develop and implement individualized plans focused on improving mobility, strength, balance, and functional independence.
• Supervise Physical Therapy Assistants (PTAs) and assist the nursing department with the training of Restorative Aides.
• Communicate regularly with physicians, nursing staff, and families regarding progress, goals, and discharge planning.
• Complete timely and accurate clinical documentation in compliance with facility, state, and federal requirements.
• Recommend and facilitate the ordering of durable medical equipment (DME) such as walkers, canes, and splints to enhance resident independence.
• Provide in-service training for facility staff and educate families on fall prevention and home exercise programs.
