Job summary

Lake Huron Medical Center in Port Huron, MI seeks a Physical Therapist I for full-time daytime hours. You will evaluate patients, develop customized treatment plans, and provide interventions to improve mobility, strength and functional independence. You will educate patients and families and coordinate with the care team to optimize outcomes.

The role requires PT licensure in Michigan, BLS on hire, and active engagement with evidence-based practice to ensure safety and quality care.

Qualifications

• Bachelor’s, Master’s, Doctorate, or certificate in Physical Therapy.
• Current state Physical Therapy license.
• Current BLS (AHA) certification on hire and maintained.
• Knowledge of physical therapy principles and patient care practices.
• Strong communication, patient education, and clinical documentation skills.

Responsibilities

• Evaluate patients’ physical function and rehabilitation needs.
• Develop and implement individualized PT treatment plans.
• Provide interventions to improve strength, mobility, balance, endurance, and independence.
• Educate patients and caregivers on exercises and recovery strategies.
• Monitor progress and adjust treatment plans to meet goals.
• Collaborate with physicians, nurses, and other healthcare professionals.
