Qualifications

• Graduate of an American Physical Therapy Association accredited school of Physical Therapy.
• Current state licensure to practice physical therapy.
• CPR certification including BLS/AED training.

Responsabilités

• Evaluate all patients referred for physical therapy and document findings.
• Complete and submit evaluation reports and progress notes promptly.
• Communicate findings and patient needs with referral sources and other departments.
• Ensure therapy practices align with APTA ethics and standards of practice.
• Maintain patient-centered care with shared decision making and informed consent.
• Maintain caseload including visits, scheduling, and documentation per policy.
• Prepare confidential department and patient reports and ensure medical necessity.

Current state PT licensure
