Key Responsibilities

• Conduct thorough evaluations of patients, including the review of medical history and relevant assessments, to develop tailored treatment plans.
• Clearly communicate evaluation findings and treatment plans to patients and their families, actively involving them in setting rehabilitation goals.
• Implement and adapt treatment protocols based on initial evaluations and changes in patient conditions, utilizing evidence-based practices.
• Maintain open communication and collaborate with other healthcare professionals to ensure comprehensive care and timely modifications to treatment plans when necessary.
• Document patient care activities accurately and promptly, including progress notes and discharge summaries, to maintain continuity in care and comply with regulatory standards.
