The position is based onsite at our state-of-the-art Rehabilitation Center in Winter Haven, Florida. Key Responsibilities

• Conduct comprehensive patient evaluations by reviewing medical history, performing assessments, and documenting findings to establish individual treatment plans.
• Effectively communicate evaluation results and treatment plans to patients and their families, actively involving them in the goal-setting process.
• Demonstrate expertise in the application of patient-specific treatment protocols, continuously monitoring and adjusting therapeutic techniques based on the patient's response and presenting condition.
• Collaborate with a multidisciplinary team, consulting with physicians and other healthcare providers as necessary to address changes in patient condition and optimize care plans.
• Prepare detailed discharge plans, ensuring continuity of care by providing patients with clear follow-up instructions and resources.
• Maintain accurate, timely documentation of patient care activities, ensuring compliance with evolving healthcare regulations and standards.
