Job summary

Westgate Gardens Care Center in Visalia, CA is seeking a full-time Physical Therapist Assistant (PTA) to join our rehabilitation team under the supervision of a licensed Physical Therapist. You will deliver skilled treatment to improve residents' mobility, balance, and independence.

The ideal candidate is clinically strong, resident-focused, and able to collaborate with an interdisciplinary team in a skilled nursing setting.

Qualifications

Associate degree from PTA program required.
California PTA license in good standing required.
Current CPR certification as required by facility policy.
Skilled nursing, long-term care, geriatric or post-acute rehab experience preferred.
Knowledge of safe resident-handling and fall-prevention techniques.

Responsibilities

Provide physical therapy treatment according to the supervising PT's plan of care.
Carry out therapeutic exercise, transfer, gait, and balance training.
Monitor residents' responses and report progress to supervising PT.
Complete timely, accurate, and compliant documentation.
Communicate with physicians, staff, residents, and families regarding progress and discharge planning.
Engage in resident-care conferences and interdisciplinary discussions as needed.
Educate residents and caregivers on goals, home programs, and safety techniques.
Assist with durable medical equipment orders to support mobility.
