Job summary

Westgate Gardens Care Center is seeking a Full-Time Physical Therapist Assistant (PTA) to join our rehabilitation team. Working under a licensed Physical Therapist, you will provide skilled therapy to help residents improve mobility, balance, and independence.

The role requires an Associate degree in PTA, California PTA licensure, CPR certification, and experience in geriatric or post-acute rehab. Join a collaborative interdisciplinary team in a skilled nursing setting.

Qualifications

• Associate degree from an accredited PTA program is required.
• Current California PTA license in good standing is required.
• CPR certification as required by policy/licensure.
• Skilled nursing or long-term care experience preferred.
• Knowledge of safe resident handling and fall prevention.
• Proficiency with Net Health and PointClickCare.

Responsibilities

• Provide physical therapy treatment per supervising plan of care.
• Carry out therapeutic exercises, transfer and gait training, balance activities.
• Monitor residents and report progress to the supervising PT.
• Document treatment timely and accurately.
• Communicate with physicians, nursing staff, and rehab team.
• Educate residents and families on goals, home exercise, safety.
• Assist with staff training and equipment needs.
• Maintain a safe treatment environment and report issues promptly.
