Responsibilities

• Conduct screening and evaluation of patients and develop care plans.
• Deliver rehabilitative, skilled and medically necessary interventions.
• Assist with transfers, ambulation, exercises and wound care as needed.
• Provide counseling to patients and families regarding physical disorders.
• Generate discharge summaries in compliance with regulations.
