Responsibilities

• Screen and evaluate patients with physical functioning disorders; develop care plans.
• Deliver skilled rehab and wound care interventions per clinical standards.
• Assist with ambulation, therapeutic exercises, modalities and wound care.
• Provide patient and family counseling regarding physical disorders.
• Prepare comprehensive discharge summaries compliant with regulations.
• Assist with coordination of care with other providers.
