Northwell Health in New York seeks a licensed Physical Therapy Assistant to support rehabilitation care plans and monitor patient progress under the supervision of a Physical Therapist. You will help maintain medical records and communicate with other health professionals.

Responsibilities include direct patient care, accurate documentation, progress notes, and participation in safety and quality activities.

Qualifications

• Associate's Degree in Physical Therapy Assistant or equivalent.
• Current NY State license to practice as a Physical Therapy Assistant.
• Outpatient setting experience with patient care is preferred.

• Submits required documentation per department policies.
• Maintains progress notes and discharge summaries for assigned patients.
• Responds to department emergency procedures for patient safety and care when necessary.

• May attend family meetings pertaining to the welfare and management of assigned patients.
• Performs related duties as required; essential functions under the ADA.

Job Responsibility

• Administers direct patient care in accordance with the written and signed prescription of the referring physician or other State authorized practitioner, as directed.
• Submits required documentation in accordance with the policies and procedures of the department.
• Maintains progress notes and discharge summaries of assigned patients.
• Responds appropriately to established department emergency procedures for patient safety and care when necessary.
• Participates in quality improvement chart reviews and activities.
• May attend family meetings/conferences pertaining to the welfare and management of assigned patients.

Job Qualification

• Associate's Degree required, or equivalent combination of education and related experience.
• Current License to practice as a Physical Therapy Assistant in New York State, required.

OR

Current limited permit to practice as a Physical Therapy Assistant in New York State, required (applicable to outpatient settings only).

Specialized certifications, as needed.
