Job summary

Northwell Health is seeking a Licensed Physical Therapy Assistant to assist in administering therapy care programs and monitor patient progress under the supervision of a Physical Therapist. You will maintain medical records for each patient and participate in department activities.

Responsibilities include documenting progress, adhering to physician prescriptions, and ensuring patient safety through established emergency procedures.

Qualifications

• Associate's Degree required, or equivalent combination of education and related experience.
• Current License to practice as a Physical Therapy Assistant in New York State, required.
• Current limited permit to practice as a Physical Therapy Assistant in New York State, required (outpatient settings only).
• Specialized certifications, as needed.
• 1-3 years of relevant experience, required.

• Submits required documentation in accordance with the policies and procedures of the department.
• Maintains progress notes and discharge summaries of assigned patients.
• Responds appropriately to established department emergency procedures for patient safety and care when necessary.
• Participates in quality improvement chart reviews and activities.
• May attend family meetings/conferences pertaining to the welfare and management of assigned patients.
