Job summary

Northwell Health seeks a licensed Physical Therapist to evaluate and treat patients with disabilities, injuries, and diseases under physician prescription. You will maintain complete medical records and provide direct patient care per orders.

The role requires a Doctoral Degree in Physical Therapy, NY State license (or eligible limited permit for outpatient settings) and 3 years of relevant experience. Travel to sites in Manhasset, Massapequa, Glen Head, Babylon, Bayshore and Lake Success is

• Doctoral Degree in Physical Therapy, or equivalent combination of education and related experience.
• Current License to practice as a Physical Therapist in New York State, required.
• Current limited permit to practice as a Physical Therapist in New York State, required (outpatient settings). Special certifications as needed.
• Eligible for NYS 90-day post-doctoral graduation practice exemption under direct on-site supervision; permit must be current before the exemption period expires.
• 3.0-3 years of relevant experience, required.

Responsibilities

• Administers direct patient care in accordance with the written prescription of the referring physician or other State authorized practitioner.
• Utilizes appropriate evaluation techniques with efficiency and thoroughness.
• Designs a treatment program to achieve established goals.
• Monitors patient tolerance/status during treatment and modifies programs as needed.
• Responds to established department emergency procedures for patient safety and care.
• Submits required documentation per department policies.
• Participates in quality improvement chart reviews and activities.
• Attends family meetings/conferences and collaborates with inter-disciplinary staff.
• Performs related duties as required; all are essential under the Americans with Disabilities Act.

Maintains complete medical records for each patient.

8. 8.Attends family meetings/conferences and interacts with members of inter-disciplinary staff pertaining to the welfare and management of assigned patients.

Job Qualification

4. Current limited permit to practice as a Physical Therapist in New York State, required (applicable to outpatient settings only).Specialized certifications, as needed.

6. Eligible for the NYS 90-day post-doctoral graduation practice exemption under direct on-site supervision. All state filings for the Limited Permit must be completed prior to hire, with a current Permit or License to practice as a Physical Therapist in New York State required before the 90-day exemption period expires.
7. 3.0-3 years of relevant experience, required.
