Job Description

The Staff Physical Therapist evaluates and treats patients/residents, communicates with families, physicians, and other health team members, and maintains documentation of services in medical records. Provides Physical Therapy services including treatment planning & implementation, and discharge planning. Tests patients’ physical abilities, analyzes rehabilitation goals, and consults with the rehab team to coordinate the most effective physical therapy program.

Organizational Relationships: Supervises: Responsible for the care delivered by PTAs and Rehab Technicians.

• Evaluate patients/residents within 24 hours of physician referral.
• Develop effective treatment plans and obtain approval from the referring physician; treat patients/residents according to the physician’s treatment plan.
• Communicate regularly with supervisor, other members of the rehabilitation team, and nursing staff regarding treatment plans, progress, and prognosis.
• Record evaluations, daily treatment notes, weekly progress notes, 14-day progress reports to the physician, and discharge summaries per policy and procedure.
• Instruct patients'/residents’ families or nursing staff in maintenance programs.
• Secure necessary durable medical equipment to facilitate independence in mobility (e.g., orthoses, walkers, canes, elevated toilet seats, grab bars).
• Submit logs and billing information in a timely manner.
• Report any issues with equipment or facilities immediately to the appropriate supervisor.
• Comply with the State Licensing Board of California, Title 22, CMS, and California Labor Board guidelines.
• Perform other duties as assigned.

Qualifications
