Job summary

Advocate Health in Illinois is seeking a licensed Physical Therapist to interpret referrals, perform comprehensive musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations, and develop individualized treatment plans.

You will monitor outcomes, adjust plans, document care, and collaborate with physicians and the care team to ensure safe, effective discharge and patient education. A $5,000 sign-on bonus is offered for qualifying full-time hires.

Qualifications

• Doctorate degree in Physical Therapy and active state licensure required.
• Basic Life Support (BLS) for Healthcare Providers within 90 days.

Responsibilities

• Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establish a plan of care and implement treatment based on evaluation results and functional goals.
• Plan for and complete patient discharge including home programs and education.
