Job summary

Functional Pathways is hiring a Full-time Physical Therapist for Westwood Ridge in West Saint Paul, MN. The PT will screen, evaluate, re-evaluate, discharge and adjust plans of care under physician supervision, ensuring high-quality resident care.

The role includes travel to multiple Vivie senior communities throughout the Minneapolis–Saint Paul area to support care across our network.

Qualifications

• Bachelor's, Master's or Doctorate degree in physical therapy from an accredited program.
• Active state license required.
• Possess current knowledge of Functional Pathways policies and procedures.
• Ability to supervise Physical Therapy Assistants.
• Excellent organizational and time management skills.
• Excellent written and verbal communication skills.
• Screening requirements may vary by location.

• Possess current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines.
• Ability to supervise Physical Therapy Assistants
• Completion of all required screenings, certifications, and licensure. Screening requirements may vary by location.
