Job summary

Heritage Community of Kalamazoo in Kalamazoo, MI is seeking a Full-time Physical Therapist to join our therapy team. The PT will screen, evaluate, re-evaluate, and revise plans of care under physician supervision, ensuring high quality patient outcomes.

Qualified candidates will have a PT degree (BS, MS, or DPT) and an active state license, with the ability to supervise PT assistants. A $10,000 sign-on bonus is offered.

Qualifications

Job Summary: The Physical Therapist shall screen, evaluate, re-evaluate, discharge plan, and make revisions in any established plan of care as supervised by the physician.

Qualifications:

• Bachelor's, Master’s or Doctorate degree in physical therapy from an accredited program.
• Certification and licensure requirements as established by state and national board associations.
• Active state license required.
• Excellent organizational and time management skills
• Excellent written and verbal communication skills
• Possess current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines.
• Ability to supervise Physical Therapy Assistants
