Job summary

ABL Health Care is seeking a compassionate Occupational Therapist to join our Home Health team in San Bruno, CA. The OT will evaluate patients, develop care plans, and monitor progress both in the field and remotely as needed.

The ideal candidate holds a CA OT license, has at least 1 year of home health or hospice experience, and can travel within a 15–25 mile radius. We offer comprehensive benefits and a team-focused environment.

Qualifications

• California OT license required
• At least 1 year of home health or hospice experience
• Valid California driver’s license
• Ability to travel within a 15–25 mile radius

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients in the home.
• Regularly evaluate and re-evaluate patient status.
• Administer treatments, monitor conditions and report changes to physicians and care team.
• Initiate, develop, implement and revise the patient’s plan of care (POC).
• Address problems identified in the POC and adjust as needed.
• Educate patients/families/caregivers regarding condition and care.
• Communicate with other disciplines involved in the case.
• Maintain accurate clinical records and documentation on company devices.
• Comply with all company policies and procedures to ensure high standards of care.
