Job summary

ABL Health Care LLC is seeking a compassionate Occupational Therapist (OT) to join our Home Health team in San Bruno, CA. This full-time position involves evaluating patients, creating and implementing care plans, and tracking progress to maintain high standards of care.

You will work with the Clinical Director and team to provide remote and field support as needed. The role requires active California OT licensure, 1 year of Home Health or Hospice experience, and a valid CA driver's license.

Qualifications

• Current OT license to practice in California
• Experience in Home Health or Hospice care (1 year)
• Current CA Driver's License
• Willingness to travel within a 15-25 mile radius

Responsibilities

• Provide, coordinate and oversee skilled intermittent care in the patients home.
• Regularly evaluates and re-evaluates patient status.
• Administer treatments, monitor patient conditions, and report changes to the physician/case manager.
• Develop and revise the patients plan of care (POC).
• Educate patient/family/caregiver about condition and care.
• Communicate with other disciplines as warranted.
• Maintain accurate clinical records using company devices.
• Follow all company policies to ensure high standards of care.
