Qualifications

• Licensed Occupational Therapist
• Experience in rehabilitation environment preferred
• Strong communication and team collaboration

Responsibilities

• Assess residents' physical, cognitive, and emotional needs.
• Develop and implement individualized therapy plans with the interdisciplinary team.
• Provide OT services including ADLs training, mobility and balance, strengthening and cognitive activities.
• Monitor progress and adjust plans as needed.
• Communicate with residents, families, and healthcare team to ensure quality care.
