Job summary

Linwood Meadows Care Center in Visalia, CA is seeking a full-time Occupational Therapist to join our rehab team. You will evaluate residents, develop individualized treatment plans, and provide interventions to improve independence in daily living skills, mobility, safety, and function.

The role requires a California OT license, a bachelor's degree, and experience with Casamba and PointClickCare. You will lead therapy sessions, collaborate with physicians and staff, supervise COTAs, document

Qualifications

• Bachelor’s degree in Occupational Therapy from an accredited program.
• California OT license in good standing.
• Master’s degree or Doctorate preferred.
• Experience with Casamba and PointClickCare preferred.

Responsibilities

• Evaluate residents and determine OT needs and rehab potential.
• Develop, implement, and modify individualized treatment plans.
• Provide interventions focused on ADLs, strength, balance, cognition, and safety.
• Communicate progress and goals with physicians, nursing staff, residents, and families.
• Participate in care plan meetings and interdisciplinary conferences.
• Document accurately and timely, as required by policy.
• Educate residents and caregivers on home programs and fall prevention.
• Supervise COTAs and therapy aides as appropriate.
