Job summary

Linwood Meadows Care Center in Visalia, CA is seeking a **Full-Time Certified Occupational Therapy Assistant (COTA)** to join our rehab team. The role works under the supervision of an Occupational Therapist to restore independence in daily living and mobility for residents.

The successful candidate will provide direct patient care, document treatments, educate patients and families, and participate in care planning while complying with regulations. Competitive pay and full benefits are offered.

Qualifications

• Associate’s degree from an accredited Occupational Therapy Assistant program (required).
• Current COTA license in good standing with the State of California.
• Experience in a skilled nursing or post-acute care setting preferred.
• Strong communication and interpersonal skills.
• Proficient in documentation software and electronic health records.

Responsibilities

• Provide direct patient care as outlined in the treatment plan developed by the Occupational Therapist.
• Assist in the implementation of treatment goals and plans.
• Monitor patient progress and report changes to the Occupational Therapist.
• Accurately document treatments and patient responses using facility systems (e.g., Casamba, PointClickCare).
• Educate residents, families, and staff on therapy goals and home exercise programs.
• Participate in team meetings, care planning, and discharge planning as needed.
• Ensure compliance with federal, state, and facility regulations and policies.
