Responsibilities

• Screen/evaluate patients with cognitive and/or ADL functioning disorders and develop a plan of care per standards.
• Use standardized assessments and evidence-based practice to guide interventions.
• Provide rehabilitative, skilled and medically necessary treatments for cognitive/ADL disorders.
• Administer procedures as part of rehabilitation including ADL retraining and therapeutic activities.
• Assess patient progress and adjust treatments in the clinical record.
• Evaluate outcomes and modify services; refer as needed.
• Provide counseling to patients and families about physical disorders.
• Communicate progress and plans with supervisor and health team members.
• Prepare comprehensive discharge summaries per regulatory requirements.
