Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our SNF team. You will evaluate and treat patients in a skilled nursing facility, following individualized plans of care and measurable goals.

You will collaborate with facility staff, document progress, re-evaluate plans, supervise OT Assistants, and educate patients and families on outcomes. This role also offers opportunities to work with a supportive team and contribute to high-quality patient care.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for your patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
