Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our team in a SNF setting. You will evaluate and treat patients following individualized plans of care, collaborating with facility staff to ensure excellent patient outcomes.

Responsibilities include developing plans of care, setting functional goals, re-evaluating progress, documenting evaluations and progress notes, educating patients and families, and supervising Occupational Therapy Assistants as needed.

• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
