Concept Rehab in St. Joseph, Michigan is seeking a compassionate Occupational Therapist (OT) to join our dedicated team on a full-time basis.

You will evaluate patients, develop individualized treatment plans and provide skilled OT services to improve independence and functional outcomes. You will collaborate with PT, SLP, nursing and other clinicians, document accurately, and help patients regain strength, mobility and daily living skills while delivering patient-centered care in a supportive

Formación

• Graduate of an accredited Occupational Therapy program.
• Current Michigan OT license or eligibility for licensure.
• Ability to work both independently and within a collaborative team.

Responsabilidades

• Evaluate patients and develop individualized OT treatment plans.
• Provide skilled OT services based on patient needs and goals.
• Help patients improve strength, coordination, mobility, ADLs and functional independence.
• Monitor progress and modify treatment approaches.
• Collaborate with PT, SLP, nursing and other members of the interdisciplinary team.
• Communicate effectively with patients, families and caregivers.
• Complete accurate and timely clinical documentation.
• Promote patient safety and positive clinical outcomes.
• Build meaningful relationships with the patients you serve.
