Job summary

Seven Oaks Rehabilitation and Healthcare Center in Columbia, SC is seeking a dedicated Occupational Therapist for a full-time position within our long-term care setting. You will evaluate residents, develop and implement treatment plans, and coordinate with physicians to ensure quality care.

Qualifications

• Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy recognized by the AOTA.
• Holds current license and/or registration in the state of South Carolina.

Responsibilities

• Provide comprehensive occupational therapy evaluation based on MD orders.
• Document findings on evaluation forms timely and accurately.
• Develop a comprehensive treatment plan with goals, frequency and duration.
• Obtain MD orders for evaluations, treatments, recertifications and discharges.
• Deliver therapy using modalities and adjust plans as needed.
• Document patient progress and update treatment plans monthly.
