Qualifications

• 3 or more years of experience of bedside patient care preferably in post-acute care with experience treating co-morbidities.

Responsibilities

• Follow up on new patient admissions adhering to the established treatment plan.
• Recertify patient stays and round on current patients.
• Update treatment plan as needed and communicate with medical staff.
