Job summary

Touchmark on West Prospect in Appleton, WI is seeking a skilled Licensed Practical Nurse to join our Health Services team. You will provide high-quality, person-centered care and support residents’ well-being within Assisted Living and Memory Care settings.

Qualifications

• LPN degree and current state licensure.
• 1–2 years of experience with older adults preferred.
• CPR/First Aid certification (or state requirements)

Responsibilities

• Provide direct nursing care within the LPN scope of practice.
• Administer medications and treatments.
• Observe, assess, and document resident conditions and care provided.
• Communicate effectively with residents, families, and the healthcare team.
• Support individualized care plans for Assisted Living and Memory Care.
• Assist with training and supporting medication aides and new team members.
