Touchmark on West Prospect in Appleton is seeking a Licensed Practical Nurse (LPN) to join our Health Services team. This full-time, non-exempt position offers direct nursing care, medication administration, and care planning for Assisted Living and Memory Care residents.

The role requires an LPN degree, current state licensure, and 1–2 years of experience with older adults. CPR/First Aid certification or ability to obtain is required, with rotating weekends and a collaborative team environment.

Qualifications

• LPN degree from an accredited nursing program with current state licensure.
• 1–2 years of experience working with older adults preferred.
• CPR/First Aid certification or ability to obtain per state requirements.

Responsibilities

• Provide direct nursing care within the LPN scope of practice.
• Administer medications and treatments.
• Observe, assess, and document resident conditions and care provided.
• Communicate effectively with residents, families, and the healthcare team.
• Support individualized care plans for Assisted Living and Memory Care.
• Assist with training and supporting medication aides and new team members.
