Job summary

Elara Caring is seeking a Licensed Practical Nurse for Behavioral Health Visits in Hartford, New Britain & Bloomfield areas. This full-time role involves following care plans, monitoring patient progress, and coordinating with the care team to deliver high-quality behavioral health visits.

Requirements include an unrestricted LPN license, nursing diploma/degree, and 1 year of clinical care experience. Must have valid driver's license or reliable transportation to travel between patient homes.

Qualifications

• 1 year of clinical care experience required.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Administer and document medications according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.
