Job description

BAYADA Home Health Care is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at the Northampton-Monroe Visits Office. This office serves adult clients on a per visit basis in homes throughout Northampton County. One year of prior clinical experience as a licensed Practical Nurse is required.

As a home care nurse, you will be a vital part of a multi-disciplinary healthcare team providing skilled nursing and rehabilitative care, enabling clients to remain at home with the necessary medical support.

• Making home visits to clients in designated areas.
• Administering medication, wound care, treatments, and procedures.
• Monitoring client conditions and reporting changes.
• Following doctors' orders and documenting care.
• Performing client assessments and case management.
• Accurately documenting observations and interventions using electronic medical records on tablets.

Qualifications:

• Current PA license as a Licensed Practical Nurse.
• Minimum of one year recent, verifiable LPN experience.
• Graduation from an accredited nursing program.
• Home care experience preferred but not required.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills and computer proficiency, EMR experience preferred.
• Ability to travel to assigned cases.
