Key Responsibilities

• The Physical Therapist will deliver high-quality, patient-centered care by managing the complete therapeutic process for an assigned group of patients, ensuring seamless coordination with physicians and care team members.
• Conduct thorough patient assessments and develop individualized care plans that reflect patient goals, focusing on engagement and satisfaction throughout the therapy process.
• Perform evaluations and implement treatment interventions that fall within the scope of practice as dictated by laws and professional standards.
• Provide purposeful and skilled therapeutic care either within the clinic or in home settings, employing evidence-based techniques to align with the physician’s plan of care.
• Instruct patients, family members, and caregivers on appropriate therapeutic exercises, use of assistive devices, and strategies for successful at-home rehabilitation.
• Manage caseload effectively by prioritizing patient needs, determining staffing requirements, and ensuring smooth transitions of care.
