This position operates predominantly in a home care setting, providing the therapist with the opportunity to deliver personalized therapy services directly to patients' residences. Key Responsibilities

• Conduct thorough evaluations of patients recovering from injury, surgery, or chronic conditions in their homes.
• Develop personalized treatment plans, utilizing evidence-based methodologies which may include therapeutic exercises, hands-on techniques, and modalities to manage pain and enhance mobility.
• Implement treatment strategies effectively and efficiently, adapting approaches as patients progress.
• Maintain meticulous records of patient treatments, responses, and progress to ensure continuity of care.
• Regularly assess and adjust therapeutic goals in collaboration with patients to tailor ongoing treatment plans effectively.
• Assist in recommending and fitting assistive devices that promote patient independence in mobility and self-care.
• Educate patients, their families, and caregivers on appropriate therapy practices, assisting them with the usage of therapeutic equipment and strategies.
• Stay updated with the latest practices in the field by participating in workshops and networking with fellow professionals.

Patient Population / Client Focus

You will be working with a diverse clientele ranging from pediatric to geriatric patients, each presenting unique challenges and recovery needs. This role requires a strong dedication to treating conditions influenced by a variety of factors, including post-operative rehabilitation, mobility impairments, neurological disorders, and orthopaedic issues. Compensation and Benefits
