Touchstone Mental Health is seeking an IRTS Behavioral and Mental Health Practitioner II / Mental Health Counselor in Fridley, MN for a full-time role. The position offers a starting pay of $55,000 with a Monday–Friday schedule, and options for 8:00 am–4:00 pm or 3:00 pm–11:00 pm shifts.

The role requires 1–2 years of behavioral health experience or 30 college credits in behavioral health, plus DHS licensing compliance.

Qualifications

• 1–2 years of experience in behavioral health or 30 college credits in behavioral health.
• Must meet the definition of Behavioral Health Practitioner per Department of Human Services licensing requirements.
• Experience with adults with SPMI, chemical dependency, and co-occurring disorders preferred.

Responsibilities

• Establish supportive relationships and coach residents to use coping strategies and daily living skills.
• Observe health symptoms, redirect maladaptive behaviors, and de-escalate crises.
• Complete intake interviews and program orientation for new clients.
• Develop discharge plans and manage the discharge process.
• Meet clients daily in individual or group sessions to support goals.
• Perform delegated nursing services under supervision.
• Ensure residents and guests follow site guidelines for safety.
• Maintain client records in an electronic health record system.
• Collaborate with staff, consultants, interns, and volunteers.

Location: Lyric Lane (Fridley)

Schedule: Monday through Friday, 8:00am-4:00pm, OR 3:00pm-11:00pm

TMH’s Behavioral and Mental Health Practitioner II provide planning, structure, and guidance to residents receiving intensive rehabilitation treatment services.

Behavioral and Mental Health Practitioner Responsibilities:

• Establish supportive and motivational relationships with residents; coach residents to utilize coping strategies and complete activities of daily living; assist in the completion of independent living skills, and provide rehabilitative skills training.
• Observe residents for changes in health symptoms; redirect maladaptive behaviors; perform behavioral management interventions; and de-escalate mental health crises.
• Complete functional assessments and intake interviews for new clients and complete program orientation.
• Develop discharge plans and complete the discharge process with clients exiting the program.
• Meet daily with clients individually or in group settings to support client goals and needs for managing their illness.
• Perform delegated nursing services, including the administration of medications, as unlicensed personnel under the direction of the nursing staff.
• Ensure residents and their guests are following site guidelines to ensure the facility is safe and secure.
• Maintain client records in an electronic health record system.
• Collaborate with other staff members, consultants, interns, and/or volunteers.

Behavioral and Mental Health Practitioner II Requirements:

• Must meet the definition of Behavioral Health Practitioner per Department of Human Services licensing requirements
• 1-2 years of experience, 30 college credits in behavioral health, or combination required
• Experience working with adults with SPMI, chemical dependency issues, and co-occurring disorders preferred
