Job summary

Favoured Hospice is seeking an experienced RN Case Manager to lead hospice care across home and facility settings, focusing on comfort and dignity. You’ll coordinate assessments, care plans, and education, while guiding patients and families through challenging times.

As a licensed RN with hospice experience, you will collaborate with an interdisciplinary team to ensure seamless, personalized care and accurate documentation. This role emphasizes compassion, reliability, and strong communication.

Qualifications

  • Graduate of an accredited school of nursing.
  • Minimum 1 year hospice case manager experience (preferred).
  • Current RN licensure in the state of practice.
  • Strong observation, verbal and written communication, and problem-solving skills.
  • Proficient in basic math and core nursing competencies.
  • Comfortable with prolonged walking or standing.
  • Able to lift, position, and transfer patients, as well as supplies and equipment.
  • Physically capable of reaching, stooping, bending, kneeling, and crouching.
  • Possess the visual acuity and hearing required to perform essential nursing tasks.
  • Must be a licensed driver with a reliable, insured vehicle that meets state and organizational standards.

Responsibilities

  • Plan, organize, and direct hospice care with a focus on comfort and dignity.
  • Collaborate with a supportive team to deliver personalized care in homes and facilities.
  • Be a guiding presence for patients and families during life’s most tender moments.
  • Conduct initial and ongoing comprehensive assessments of patients and families.
  • Perform physical evaluations and document medical history.
  • Develop, initiate, and revise individualized plans of care.
  • Use health assessment data to determine nursing diagnoses.
  • Include patients and families in the care planning process.
  • Administer medications as prescribed.
  • Provide health education and counseling to patients and families.
  • Assist with daily living activities and promote comfort care.
  • Serve as RN Case Manager for assigned caseload when designated by Clinical Supervisor.
  • Coordinate care and collaborate with team members to ensure continuity.
  • Accurately complete, maintain, and submit timely clinical notes reflecting the patient’s condition and care provided.
  • Document changes in pain and symptom management, including outcomes and interventions.
  • Communicate proactively with physicians regarding patient needs and condition changes; obtain and implement physician orders as required.
  • Coordinate care by collaborating with community health professionals and resources.
  • Provide medications, dietary guidance, and other instructions as prescribed by the physician.
  • Identify and utilize opportunities for comfort counseling with patients, families, and caregivers.
  • Deliver care in alignment with the interdisciplinary team to ensure holistic support.
  • Maintain a safe, clean, and supportive environment for patients.
  • Assist patients, families, and team members in delivering compassionate, comfort-focused care.
  • Support the emotional and psychosocial needs of patients and families in collaboration with the hospice team
  • Actively participate in interdisciplinary group (IDG) meetings to ensure coordinated, patient-centered care.

Requirements
